Please find attached a copy of the new application form for all survey request to the Marine Survey Office. This new combined application form will replace the previous version of a Surevy 6 form and the Application for a Passenger Boat licence.

Please note that only requests for survey using this application form will be entertained from 16/03/09 onwards.
